If you clicked the under 13 link, then you must await a manual activiation from an Administrator. Otherwise, you recieved an activation link in your e-mail. Clciking that link will make your account active.

We had trouble with that too. Seeing as the E4 don't have a Fax machine, your parents need to print out the form, sign it, scan it into your computer, and e-mail it the the E4.
As to where the form is, it should be in your e-mail

Keep waiting. I'm sure the admins are quite busy (last time I checked), so they'll get to activating your account after they've received and confirmed the information. While you're waiting, why don't you enjoy the wonderful site that you can use without the need of a COPPA form?
