Can Wormadam change between its alternate forms like burmy, or is it evolving into that form forever??

wormadam cannot change forms, only burmy can

Yeah, Whatever forme of Burmy you have it in when it evolves determines the Wormadam it is meant to be forever after evolving & once it does evolve, you are stuck with your choice forever
