Also, make sure you uncheck the "disable BBCode" box below the text-editing area. For whatever reason, it's checked automatically for signatures.

Upload it to a image hosting site such as imageshack or PhotoBucket, then put it into your sig like you would a normal image with the [img] tags. Make sure that it meets the standard signature requirements.
